External Publications Using GUI Data

Collecting accurate and detailed dietary intake data is costly at a national level. Accordingly, limited dietary assessment tools such as Short Food Questionnaires (SFQs) are increasingly used in large surveys. This paper describes a novel method linking matched datasets to improve the quality of dietary data collected. Growing Up in Ireland (GUI) is a nationally representative longitudinal study of infants in the Republic of Ireland which used a SFQ (with no portion sizes) to assess the intake of “healthy” and “unhealthy” food and drink by 3 years old preschool children. The National Preschool Nutrition Survey (NPNS) provides the most accurate estimates available for dietary intake of young children in Ireland using a detailed 4 days weighed food diary. A mapping algorithm was applied using food name, cooking method, and food description to fill all GUI food groups with information from the NPNS food datafile which included the target variables, frequency, and amount. The augmented data were analyzed to examine all food groups described in NPNS and GUI and what proportion of foods were covered, non-covered, or partially-covered by GUI food groups, as a percentage of the total number of consumptions. The term non-covered indicated a specific food consumption that could not be mapped using a GUI food group. “High sugar” food items that were non-covered included ready-to-eat breakfast cereals, fruit juice, sugars, syrups, preserves and sweeteners, and ice-cream. The average proportion of consumption frequency and amount of foods not covered by GUI was 44 and 34%, respectively. Through mapping food codes in this manner, it was possible, using density plots, to visualize the relative performance of the brief dietary instrument (SFQ) compared to the more detailed food diary (FD). The SFQ did not capture a substantial portion of habitual foods consumed by 3-year olds in Ireland. Researchers interested in focussing on specific foods, could use this approach to assess the proportion of foods covered, non-covered, or partially-covered by reference to the mapped food database. These results can be used to improve SFQs for future studies and improve the capacity to identify diet-disease relationships.

Background
At age 5, while only embarking on their educational journey, substantial differences in children’s cognitive ability will already exist. The aim of this study was to examine the causal association between below average cognitive ability at age 5 years and child-reported experience of school and self-concept, and teacher-reported class engagement and emotional-behavioural function at age 9 years.

Methods
This longitudinal cohort study used data from 7,392 children in the Growing Up in Ireland Infant Cohort, who had completed the Picture Similarities and Naming Vocabulary subtests of the British Abilities Scales at age 5. Principal components analysis was used to produce a composite general cognitive ability score for each child. Children with a general cognitive ability score more than 1 standard deviation (SD) below the mean at age 5 were categorised as ‘Below Average Cognitive Ability’ (BACA), and those scoring above this as ‘Typical Cognitive Development’ (TCD). The outcomes of interest, measured at age 9, were child-reported experience of school, child’s self-concept, teacher-reported class engagement, and teacher-reported emotional behavioural function. Binary and multinomial logistic regression models were used to examine the association between BACA and these outcomes.

Results
Compared to those with TCD, those with BACA had significantly higher odds of never liking school [Adjusted odds ratio (AOR) 1.82, 95% CI 1.37–2.43, p < 0.001], of being picked on (AOR 1.27, 95% CI 1.09–1.48) and of picking on others (AOR 1.53, 95% CI 1.27–1.84). They had significantly higher odds of experiencing low self-concept (AOR 1.20, 95% CI 1.02–1.42) and emotional-behavioural difficulties (AOR 1.34, 95% CI 1.10–1.63, p = 0.003). Compared to those with TCD, children with BACA had significantly higher odds of hardly ever or never being interested, motivated and excited to learn (AOR 2.29, 95% CI 1.70–3.10).

Conclusion
Children with BACA at school-entry had significantly higher odds of reporting a negative school experience and low self-concept at age 9. They had significantly higher odds of having teacher-reported poor class engagement and problematic emotional-behavioural function at age 9. The findings of this study suggest BACA has a causal role in these adverse outcomes. Early childhood policy and intervention design should be cognisant of the important role of cognitive ability in school and childhood outcomes.

Lone mothers have been found to report lower average mental health than partnered mothers. Following the ‘stress process model’, disparities in women’s mental health by family structure could be explained by lone mothers’ higher exposure to multiple forms of stressors, compared to partnered mothers. Yet, this hypothesis has not been tested in previous studies. This study analysed four waves of longitudinal data from the Growing Up in Ireland study, spanning between the year when women gave birth (2008) to 9 years later (2017) (N = 5,654 women), to examine how family stressors (i.e., financial strain, caregiving strain, work-related strain, and parental conflict) influence mothers’ depressive symptoms by family structure. Analyses applied random-effects models and Karlson-Holm-Breen (KHB) decomposition techniques, combined with different model specifications as robustness checks (i.e., fixed-effects). Results indicate that: (1) net of sociodemographic factors, lone mothers experience higher levels of depressive symptoms than partnered mothers, with additional analyses confirming that transitioning from partnered to lone mother is associated with higher depressive symptoms, and from lone to partnered mother with reduced depressive symptoms; (2) although 41% of the observed statistical association between family structure and mothers’ depressive symptoms is direct, a larger 59% of this mental health gap is mediated by inequalities between lone and partnered mothers in their exposure to family stressors; and (3) the largest share of the observed mediation by family stressors is explained by lone mothers’ higher risks of current and past caregiving strain and parental conflict, but also by their current higher financial strain. Overall, this study suggests that lone mothers’ lower mental health, compared to partnered mothers, is largely explained by disparities in exposure to family stressors, pointing to how accumulated caregiving and parental stressors, as well as poverty risks, are key explanatory factors behind the mental well-being disadvantage that lone mothers face.

The objective was to identify determinants of uptake and maintenance of active school travel (AST) over 4 years in children aged 9 at baseline. Data from wave 1 (n=8502) and 2 (n=7479) of the Growing Up in Ireland study were analysed. At 9- and 13-years 25% and 20% engaged in AST. Children were more likely to maintain or take-up AST if they lived in an urban area. Change in distance to school influenced both maintenance and adoption of AST, with a negative impact seen for increased distance between 9 and 13 years and a positive impact seen for decreased distance. Some factors which predict uptake and maintenance of AST are modifiable and can inform intervention development.

This study examined the role of neighbourhood characteristics in explaining socioeconomic inequalities in child mental health (the total difficulties score from the Strengths and Difficulties Questionnaire) using data from Cohort ’08 of Growing Up in Ireland Waves 3 (age 5; baseline) and 5 (age 9; follow-up). Twenty neighbourhood items were grouped into neighbourhood safety, built environments, cohesion, interaction, and disorder. Data were analysed using regression, single and multiple mediation, and network psychometric analyses. We found that neighbourhood safety, cohesion, interaction, and disorder were associated with child mental health. These four domains separately (by up to 18 %) or in concert (by up to 23 %) partially explained socioeconomic inequalities in child mental health. Built environments may explain socioeconomic inequalities in mental health in urban children only. Findings from network analysis indicated that specific concerns over “people being drunk or taking drugs in public” and “this is a safe neighbourhood” had the strongest connections with child mental health. Improving neighbourhood characteristics may be important to reduce socioeconomic inequalities in child mental health in Ireland.

This paper, for the first time, presents estimates of the causal impact of overweight and obesity on self-rated health (SRH) using instrumental variables (IV) econometric methods. While a number of previous studies have sought to better understand the determinants of SRH, there is no consensus in relation to the impact of overweight and obesity. Using data from a large nationally representative sample of Irish parents and their children, we estimate a range of ordered probit models to isolate the causal effect of overweight and obesity on SRH. Our data includes independently and objectively recorded weight and height measures for parents and their children and we instrument for parental body mass index (BMI) status using the BMI of a biological child. After controlling for a range of individual, socioeconomic, health and lifestyle related variables, we find that being overweight has a negligible impact on SRH, while being obese has a practically and statistically significant negative impact on SRH, with these effects most pronounced for those who are most obese. We find only minor differences in these effects across gender.

Background
Height is regarded as a marker of early-life illness, adversity, nutrition and psychosocial stress, but the extent to which differences in height are determined by early-life socioeconomic circumstances, particularly in contemporary populations, is unclear. This study examined socioeconomic differences in children’s height trajectories from birth through to 21 years of age in four European countries.

Methods
Data were from six prospective cohort studies—Generation XXI, Growing Up in Ireland (infant and child cohorts), Millennium Cohort Study, EPITeen and Cardiovascular Risk in Young Finns Study—comprising a total of 49 492 children with growth measured repeatedly from 1980 to 2014. We modelled differences in children’s growth trajectories over time by maternal educational level using hierarchical models with fixed and random components for each cohort study.

Results
Across most cohorts at practically all ages, children from lower educated mothers were shorter on average. The gradient in height was consistently observed at 3 years of age with the difference in expected height between maternal education groups ranging between −0.55 and −1.53 cm for boys and −0.42 to −1.50 cm for girls across the different studies and widening across childhood. The height deficit persists into adolescence and early adulthood. By age 21, boys from primary educated maternal backgrounds lag the tertiary educated by −0.67 cm (Portugal) and −2.15 cm (Finland). The comparable figures for girls were −2.49 cm (Portugal) and −2.93 cm (Finland).

Conclusions
Significant differences in children’s height by maternal education persist in modern child populations in Europe.

This nationally representative study of Irish infants explores whether the set of child and environmental factors established as predicting language outcomes aged 3 years would also predict language and communication development as early as age 9 months. Associations between infant and environmental characteristics and infant language outcomes at 9 months, as measured on the ASQ 10-month communication questionnaire, were explored in a series of bivariate logistic regression models and in a fully adjusted multivariable logistic regression model. Infant gender, health, birthweight and temperament were significantly associated with passing the ASQ communication measure at 9 months. Being an only child and having some experience of relative childcare predicted positive communication outcomes. Infants of older mothers and mothers who spoke less to the child while doing other things were significantly less likely to pass. Infants of mothers with the lowest educational level were more likely than their peers to pass at 9 months. Unlike language outcomes at 3 years, low maternal education does not appear to be a risk factor for poorer outcomes at 9 months. This study adds significantly to the infant language development literature by focussing on communication outcomes in early infancy and identifying protective and risk factors at such an early stage.

Gender inequality in housework divisions is persistent. This study examined early-emerging patterns in gender roles via children’s gendered housework time in the Irish context using time-use diary and questionnaire data, controlling for key demographic variables. This study utilized longitudinal “Growing up in Ireland” data, comparing the same children’s behaviour at age 9 and 13. Participants were a nationally representative, weighted sample of school going children living in Ireland, including 4,135 girls, 3,831 boys and their caregivers. The results highlight that inequalities in domestic chores occur at nine and are even greater at 13: girls do more housework and children tend to do “gender typical” chores. Consequences are discussed in relation to gender inequality and policy implications.

Children born preterm have poorer outcomes than children born full-term, but the caregiving environment can ameliorate some of these differences. Recent research has proposed that preterm birth may be a plasticity factor, leading to better outcomes for preterm than full-term infants in higher quality environments. This analysis uses data from two waves of an Irish study of children (at 9 months and 3 years of age, n = 11,134 children) and their caregivers (n = 11,132 mothers, n = 9998 fathers) to investigate differences in how caregiving affects social, cognitive, and motor skills between full-term, late preterm, and very preterm children. Results indicate that parental emotional distress and quality of attachment are important for child outcomes. Both being born very preterm and late preterm continue to be risk factors for poorer outcomes at 3 years of age. Only fathers’ emotional distress significantly moderated the effect of prematurity on infants’ cognitive and social outcomes—no other interactions between prematurity and environment were significant. These interactions were somewhat in line with diathesis stress, but the effect sizes were too small to provide strong support for this model. There is no evidence that preterm birth is a plasticity factor.

A Tobacco-Free Future – An all-island report on tobacco, inequalities and childhood 2013 reveals declines in smoking rates among both children and pregnant women over the past decade, both North and South of the border. This report published by the Institute of Public Health in Ireland (IPH) and the TobaccoFree Research Institute Ireland (TFRI), shows that while tobacco control measures are being successful, disadvantaged children are at particular risk of tobacco-related harms.

Children growing up in disadvantaged circumstances face a number of threats to their health and development. Protecting children from the burden of tobacco related harm from both active and passive smoking is a priority action in enhancing population health and reducing health inequalities. Population health strategies on the island of Ireland are increasingly focussing on addressing the root causes of health inequality through social determinants of health approaches and through focussing on early childhood as a key period for intervention. At the same time, governments in both jurisdictions are working to enhance their approaches to effective tobacco control.

The World Health Organization considers that there are three key ‘windows of exposure’ in terms of tobacco-related harm in childhood – in the womb (associated with active or passive smoking by the mother), directly through children taking up smoking and through exposure to second hand smoke (SHS) in indoor and outdoor environments.

This report presents findings on these three windows of exposure based on a range of data sources in the Republic of Ireland and Northern Ireland. The central aim of the report is to contribute to knowledge on the exposure of children to the harmful effects of tobacco smoke at various stages of their development. The findings of the report can support policy makers and service providers in their efforts to make tobacco-free childhoods a reality on the island of Ireland.

The paper examines the effects of socioeconomic background (SES) – measured by social class, family income and parental education – cognitive ability, and gender on a variety of key outcomes from a large longitudinal study based on a representative sample of thirteen-year-olds. The data analysed comprised 6216 children who participated in waves 1 to 3 of the Growing Up in Ireland (GUI) longitudinal survey. The outcome measures drawn from wave 3, when respondents were aged about seventeen, were: examination results and several cognitive measures, life difficulties, and quality of relationships. Three regression models were compared with and without, SES measures (occupational class, household income and parental education) and cognitive ability. On academic and cognitive attainments, cognitive ability at age 13 had substantially more explanatory power than the SES measures together. On measures of adolescent difficulties and on family relationships, cognitive ability was important, but gender and to a lesser extent, household income and parental education had some effects. Claims that class background and family income are of central importance for adolescent outcomes are not supported.

Technological advancement has seen a rise in computer-based and online gaming, and the online space has facilitated a proliferation in gambling options that increasingly adopt game-like features. There is a concern that gaming in childhood could predispose subsequent engagement with gambling behaviors, which has drawn the attention of parents, researchers, governments, and the European Union. To inform policy and legislation aimed at mitigating gambling-related harms, this research uses three waves of longitudinal data from Ireland to examine whether computer game engagement at 9-years-old (collected in 2007/8), and online gaming at 17 and 20, is associated with gambling participation in young adulthood. Multiple regression models, adjusting for a range of socio-economic and health characteristics, revealed that online gaming at 17 was associated with 1.4 higher odds of more frequent online gambling at 20 years (p = 0.011), while online gaming at 20 was associated with a 1.7 higher odds (p < 0.001). However, no association between early computer-based gaming at 9 years and subsequent gambling behaviors at 20 was uncovered for this cohort (born in 1998). Given more recent introductions of loot boxes and social casino games, continuous research is needed for future cohorts which may be greater affected by such developments.

The internet provides opportunities for social interactions which first occur in an online environment that can lead to meeting up in real life. However, growing concerns around safety and privacy warrant greater study of this modern-day phenomenon. Using a longitudinal dataset of approximately 4,300 20-year-olds in Ireland, this study finds one-in-three report meeting someone from online. Multiple regression reveals that females are less likely to engage in such behaviour, while the trait of ‘openness’, spending more time on the internet, being non-heterosexual, using dating apps, and being sexually active at 17 years are positively associated. Early exposure to information and communication technologies, as indicated by mobile phone ownership at 9 years, is also associated with online-to-offline encounters. A range of policy considerations are discussed, including the continued need for education in e-safety and cyber safety awareness from a young age through young adulthood, particularly for groups such as LGBT communities.

This paper presents an analysis of primary schoolchildren’s attitudes to the Irish language, Gaeilge, in the context of national policy in the Republic of Ireland. In particular, the study examines the factors (social, cultural, cognitive and organisational) that may be related to a pronounced excess in disengagement with Irish over and above general engagement with schooling. The data derives from the nine-year-old child cohort of the national longitudinal study of children, the Growing Up in Ireland (GUI) survey. Unlike previous findings in relation to general engagement with school, excess disengagement with Irish is not found to be related to socio-economic or ethnic factors, factors related to teacher profile, training, self-efficacy or style or school ethos. As expected, children with less exposure to spoken Irish in school or at home are more likely to show excess disengagement with the language. Literacy activity, special education needs and school patronage are also found to be related to excess disengagement. Significant random effects indicate the importance of the individual teacher and school in the development of pupil engagement with Irish. The current study presents a timely perspective on attitudes to Irish among the younger population and possible implications for policy.
